Page setup for a double page spread with facing pages:

The first two pages of any 'facing page' documents can never be shown next to one another.

Print settings: 
Select spreads
Marks and bleed - ensure the bleed is going to print
If working with bleed also print the document with crop marks showing
Specific pages can be chosen for print if 'range' is selected rather than 'all'

Enabling pages to be placed next to one another in non-facing page documents.
Select the drop down menu on the right of the pages pallet.
Deselect the 'Allow Document Pages to Shuffle' option

Once deselected, pages can be rearranged into a preferred, more specific range of layouts.
The pages can stand alone or be designed as two-sided page designs as the document is not set as a facing page document.
